What it is used for

The National Institutes of Health Stroke Scale is the standard measure of neurological deficit in acute stroke. Fifteen items are scored and summed to a maximum of 42, giving a reproducible severity figure that guides reperfusion decisions and lets change be tracked between assessments. In the UK, NICE NG128 uses an NIHSS of 6 or more as the point at which thrombectomy is considered where a large vessel occlusion is confirmed on CT angiography. Thrombolysis is a separate decision governed by the 4.5-hour window and its own contraindications.

How the score is interpreted

0
No deficit
No neurological deficit detected. Reassess — consider TIA, Todd's paresis, or a stroke mimic.
1–4
Minor stroke
Minor stroke or TIA. Aspirin 300 mg stat if haemorrhage has been excluded, urgent brain imaging, stroke team.
5–15
Moderate stroke
From 6 upwards, consider thrombectomy if a large vessel occlusion is confirmed on CTA — up to 24 hours in selected patients (NICE NG128). Thrombolysis if within 4.5 hours and no contraindications.
16–20
Moderate to severe stroke
Urgent stroke team activation. Consider thrombectomy if LVO is confirmed, and thrombolysis if within the window.
21–42
Severe stroke
Urgent stroke team activation, reperfusion assessment, and consideration of neurocritical care.
